Testosterone


We prescribe a dose of testosterone that gets you and keeps you within your normal and healthy ideal range, which means your average total and free testosterone levels are above the reference range – where they’re supposed to be!

Anastrozole


As men age their testosterone levels plummet and their cells decrease their number of testosterone receptors. The disappearance of testosterone receptors means that whatever testosterone is around is more likely to be stuck outside of the cell and therefore be converted into estrogen. If men as they age are gaining any extra weight especially in the mid-section, if they’re under chronic stress, drinking a significant amount of alcohol, or using cannabis, this process of conversion of testosterone to estrogen gets even worse, and this whole process becomes a vicious positive feedforward cycle where testosterone levels further drop, testosterone receptors continue to disappear, and estrogen levels increase.

If men with low testosterone forego TRT for low testosterone, then this disappearance of testosterone receptors gradually becomes irreversible beginning around age 50 or so and over the course of the next about 20 years up to about 70 years of age. In the mean time, it’s reversible, but it takes short-term supersaturation of the testosterone receptors by testosterone for the cells to increase their number of testosterone receptors again. What this means is that when men with low testosterone initiate testosterone replacement therapy (TRT) they will initially require a significant amount of an aromatase inhibitor to prevent the conversion of testosterone to estrogen. Over time, the required dose of anastrozole usually decreases, but it can be quite significant for some time. Of course, in cases where men become a lot more physically active and improve their body composition, testosterone doses can increase such that the anastrozole dose remains the same or even increases. This is why it’s important to make sure you’re going to a specialty TRT clinic where you can receive testosterone replacement therapy under the supervision of a team of doctors who are TRT experts.

If testosterone is prescribed without the appropriate anastrozole dose, then an overwhelming amount of estrogen will be produced from the prescribed testosterone, which will outcompete testosterone for testosterone receptors, and this pretty much causes the opposite of what men with low testosterone want from testosterone replacement therapy (TRT). Trust me, there are better ways for men to get in touch with their feminine side. Without the appropriate prescription of the just-right amount of anastrozole by a Seattle TRT doctor who is an expert at TRT for low testosterone there’s no way to normalize testosterone levels without causing overwhelming estrogen levels.

Our Seattle TRT doctors prescribe a dose of anastrozole that gets and keeps men’s estrogen levels within their normal and healthy ideal range, which means that they get all of the benefits of having a normal amount of estrogen, without suffering any adverse effects of having either too much or too little estrogen. This is crucial, because estrogen is actually a very important hormone for men! Men will not feel normal and healthy if their estrogen levels are either too high or too low. An example of how important estrogen levels are is that if men have too much estrogen, then they can’t achieve a firm erection, and if they have too little estrogen, then they can’t maintain one. I think that’s all I have to say about estrogen for men to understand it’s importance, but there’s a whole lot more to it than that!

Gonadorelin or Human Chorionic Gonadotropin (HCG)


Neither gonadorelin nor HCG is a required part of TRT. We only recommend the ongoing use of one of these medications for patients who want to prevent gradual testicular shrinkage, and a gradual loss of their ability to make testosterone and sperm while they’re on TRT. This will gradually happen without the ongoing use of one of these medications, because otherwise when your testosterone levels are kept in an ideal range, then over time your hypothalamus and anterior pituitary gland will determine that there’s no need for them to produce their stimulatory hormones, so over time they gradually weaken and shrink, and the entire pathway for making testosterone and sperm will shut down.

Either of these medications can work very well to accomplish this. Gonadorelin is preferred when then anterior pituitary is able to respond to it. Gonadorelin is another name for gonadotropin releasing hormone (GnRH), which is made by the hypothalamus in the brain. Gonadorelin therefore stimulates the anterior pituitary to make and release luteinizing hormone (LH), follicle stimulating hormone (FSH), and to a much lesser extent, human chorionic gonadotropin (HCG). LH then stimulates the testes to make testosterone, while FSH then stimulates the testes to make sperm, while HCG then stimulates the testes primarily to make testosterone, but secondarily also stimulates the testes to make sperm.  HCG is preferred when the anterior pituitary is not able to respond to gonadorelin. In this case, it makes the most sense to bypass the anterior pituitary and directly stimulate the testes with HCG. Based on our experience, patients are able to maintain most if not all of their testicular functional capacity by taking one of these medications while on TRT.
